business.PROGRAM SUMMARYAction Central (Assertive Community
Treatment) is a dual-track treatment program for individuals with
severe mental illness and/or a chronic substance use disorder. The
program provides therapeutic interventions, peer support,
self-management skills, medication management and monitoring,
supportive housing services, comprehensive outpatient and community
services, and more.JOB SUMMARYThe AOD Case Manager assists clients
with housing access and access to social and community support
systems that provide AOD treatment and recovery opportunities. Acts
as a liaison between treatment providers, ancillary services,
landlords and the client. Conducts intake and needs assessments,
arranges and coordinates comprehensive services, develops treatment
and recovery plans, with referrals to AOD treatment as part of the
client plan. Supports clients to achieve stabilization and enables
clients to maintain housing. Advocates for clients through
community outreach and extensive knowledge of community resources
to assist each client with achieving long-term recovery, housing
and employment.RESPONSIBILITIESReasonable accommodations may be

perform assessments of client status and level of comprehensive
services required.Prepare service plans.Maintain all progress
notes, records and documentation.Promote Housing First
ideologies.Act as a liaison between client and treatment providers,
ancillary services, and landlords.Facilitate housing placements
including arranging pre-placement visits and transportation.Comply
with all MHS and program rules, policies and procedures including
HIPAA standards.QUALIFICATIONSBachelor's Degree or Associates
degree preferred, High School/GED requiredCertification or in
process of obtaining certification for substance abuse treatment,
such as CAADAC, CASS or similar certificationMinimum of two years
providing substance abuse treatment and care to adultsDemonstrated
in-depth knowledge of substance abuse treatment methodologies and
diagnostic techniques. Familiarity with San Diego substance abuse
services systemAbility to communicate verbally and in
writingProficient with Microsoft Office Suite and EHR
softwareAbility to problem solve and react appropriately to
emergency situationsAbility to work independently and effectively
collaborate with a teamValid California driver's license and
current auto insurance coveragePHYSICAL REQUIREMENTSThe physical
