ROLE OVERVIEW & PURPOSE The LVN-care management role reports to the Quality Manager, and its providers for rendering nursing care in assigned area, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing. The LVN-care coordinator role is also part of the patient-centered medical home (PCMH) and assists with coordinating and providing panel management services to patients within the practice who are at most risk for health deterioration or poor outcomes. When providing direct patient care, the LVN schedules educates patients on chronic disease management and empowers patients to meet their self-care management goals. The LVN works with the care team to increase patient's access to appointments, services, and health care resources as well as enhances communication between patients and practitioners to facilitate continuity of care. PERFORMANCE AREAS ESSENTIAL DUTIES/RESPONSIBILITIES: 1. Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes. 2. Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards. 3. Carry out direct and indirect provider orders accurately. 4. Document pertinent observation and intervention on appropriate record. 5. Perform selected tasks related to patient care and the patient's environment in a safe manner. 6. Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes. 7. Perform limited I.V. Therapy procedures on those patients receiving peripherally inserted Intravenous fluids, as delegated by and observed the RN. 8. Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback. 9. Provide patient/significant other health teaching as delegated by and observed by the RN. 10. Participate in staff development programs as required by Neighborhood Healthcare. 11. Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. 12. Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated. 13. Assist in the orientation/training of new Medical Assistants. 14. Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns. 15. Consistently evaluate work and determine if further steps are needed to meet client expectations. 16. Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals. 17. Keep self and others informed of organizational activities and promote mission and goals. 18. Function at highest level according to credentials and competency 19. Positively impact patient experience by demonstrating values of Transforming Care including, but not limited to, courteous and helpful behavior and a commitment to accuracy 20. Share accountability for overall patient health outcomes, working in coordination with Care Teams 21. Operate to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders 22. Care manage assigned patients for preventative and chronic disease programs including but not limited to hypertension, diabetes, annual wellness visits. 23. Develop care plans for patient/significant other based on identified problems/needs. 24. Other duties as assigned. TEACHING/EDUCATION 1. Uses a step-by-step process when teaching skills. 2. Uses positive reinforcement and encouragement. 3. Uses a flexible approach with a diverse population. 4. Attends both mandatory in-service trainings and related meetings, providing feedback to staff on the content of educational programs attended. 5. Coaches patients to address critical issues using motivational interviews and helps patients develop achievable self-management care plan goals. 6. Reinforces information given to the patient and/or family with handouts to improve patient self-management skills and communication. 7. Provides as-needed teaching to patients during medical appointments. RECORD KEEPING 1. Completes progress notes for all care coordination and other services provided in a timely manner. 2. Monitors and maintains confidentiality in accordance with Neighborhood's policy and legal requirements. 3. Prioritizes activities according to need and required follow-up. 4. Provides accurate and timely reports to supervisor as required. QUALITY MANAGEMENT: 1. Display knowledge of normal signs of human development and ability to assess and provide age appropriate care. 2. Contribute to the success of the organization by participating in quality improvement activities including grant and care innovation projects. PHYSICAL REQUIREMENTS: 1. Fine hand manipulation, Picking up, typing or otherwise working, primarily with fingers rather than with the whole hand as in handling. 2. Feeling. Perceiving attributes of objects, such as size, shape, temperature or texture by touching with skin, particularly that of fingertips. 3. Talking, Expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly. 4. Hearing. Perceiving the nature of sounds at normal speaking levels with or without correction. Ability to receive detailed information through oral communication, and to make the discriminations in sound. 5. Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. 6. The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection); using measurement devices; and/or assembly or fabrication parts at distances close to the eyes. 7. The worker is not substantially exposed to adverse environmental conditions (such as in typical office or administrative work.) COMPLIANCE CUSTOMER RELATIONS 1. Respond promptly and with caring actions to patients and employees. Acknowledge psychosocial, spiritual and cultural beliefs and honor these beliefs. 2. Maintain professional working relationships with all levels of staff, clients and the public. 3. Be part of a team and cooperate in accomplishing department goals and objectives SAFETY 1. Maintain current knowledge of policies and procedures as they relate to safe work practices. 2. Follow all safety procedures and report unsafe conditions. 3. Use appropriate body mechanics to ensure an injury free environment. 4. Be familiar with location of nearest fire extinguisher and emergency exits. 5. Follow all infection control procedures including blood-borne pathogen protocols HIPAA/COMPLIANCE 1. Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes. 2. Comply with all regulations regarding corporate integrity and security obligations. Report unethical and fraudulent or unlawful behavior or activity. QUALIFICATIONS Emotional intelligence (EI): the ability to be sensitive to and understand the emotions of others and to manage his or her own emotions and impulses required. Effective verbal and written communication, organization and interpersonal skills required. Ability to interface with all levels of personnel in a professional manner. Ability to work with people of all social and ethnic backgrounds and maintain confidentiality. Ability to work independently and as part of a team. Bilingual (English/Spanish or Arabic) preferred. Familiarity with basic word processing, spreadsheet, and database applications. Accurate keyboarding skills. Skills: Valid registration as a LVN in the State of CA and current CPR.
