staff.Provides case review to patients within assigned community
health program, ensuring the medical program is clinically correct
and being followed.Responsibilities:Completes all required
documentation accurately, in a timely manner, and thoroughly in
accordance with department standards; in addition, ensures that all
client files are kept up to date with RN signature on required
documents on a monthly basis.Completes Chart Reviews, ensuring
medical program requested is being followed.Ensures applicable
regulatory and department standards are adhered to on an on-going
basis (The Joint Commission, OSHA, etc.).Performs other duties as
assigned.Reviews and sign-off on all Case Managed client Care
Plans, Initial Assessments, and Comprehensive
Assessments.Requirements:1 year of work experience providing human
services to high risk, medically underserved, disabled populations,
and/or relevant community health populations required.Current
California Registered Nurse licensure. Nurse Practitioner,
Physician Assistant, or Medical Doctor licensure accepted.Or
equivalent combination of education and experience that provides
the skills, knowledge and ability to perform the essential job
duties, and which meets any required state or federal certification
requirements.Current American Heart Association healthcare provider
CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life
Support, or Advance Resuscitation Training required.Ability and
means to travel as needed in a timely manner within San Diego
County, to locations that may have limited access to public
transportation; proof of liability and property damage insurance on
vehicle used is required.DRIVER REQUIREMENTS: Age 21; Licensed for
a minimum of 3 years; No violations and/or accidents within 3
years; No vehicle related suspensions/reinstatements; No DUI,
reckless or felony Driving with 7 yearsRewards:Job type: Regular

spirit of pay transparency, we are excited to share the base range
for this position, exclusive of fringe benefits or potential
bonuses.$40.00 - $49.95If you are hired at Family Health Centers of
San Diego, your final base salary compensation will be determined
based on factors such as geographic location, jurisdictional
requirements, skills, education, and/or experience. In addition to
these factors - we believe in the importance of pay equity and
consider internal equity of our current team members as a part of
any final offer. Please keep in mind that the range mentioned above
is what we reasonably expect to pay for the role. Hiring at the
maximum of the range would not be typical in order to allow for
