Job Description:
A company is looking for a Service Support Facilitator to join
their team.
Key Responsibilities
Schedule, conduct, and document member assessments within
prescribed timelines
Enter assessment data into the web system and participate in
inter-rater activities
Conduct educational sessions for family members and agencies
regarding assessments and budgetary policies
Required Qualifications
Bachelor's Degree in Human Services (social work, counseling, or
psychology)
Minimum of 2 years of experience with children's mental health
Must live in the State of West Virginia
