A company is looking for a Care Coordinator, Per Diem - Sexual
and Reproductive Health.
Key Responsibilities
Collaborate with a multi-disciplinary team to ensure a high-quality
patient experience
Ensure adherence to processes and procedures for efficient delivery
of care
Manage patient/provider messaging and coordinate treatment and
community resources
Required Qualifications
One year of experience in a medical practice with knowledge of
medical terminology
Willingness to work regular weekends and at least three holidays a
year
Self-motivated with a service-oriented mindset
Skilled in using online tools and technology for communication and
care delivery
Reliable attendance and adherence to the assigned work schedule
